Adam Air Flight 574 Plunged Into Sea

VIVAnews - 2 years ago today, on 1 January 2009 an Adam Air Boeing 737 crashed into the sea near Polewali, Sulawesi. All 102 people on board were killed.

Adam Air Flight DHI-574 disappeared from radar on New Year’s Day, 2007, during a scheduled domestic flight between Surabaya on the island of Java to Manado on the island of Sulawesi.

The aircraft, a Boeing 737-4Q8 was manufactured in 1990 and already had 45,371 flying hours. It was last evaluated and declared airworthy by the Indonesian Department of Transportation on 25 December 2005. The plane was due to be checked again in late January 2007, only days prior to its crash.

After an extensive search on land and at sea, some debris of the aircraft was found in the sea nine days after the accident. The plane was ultimately determined to have crashed into the ocean and all 102 people on board were presumed dead.

The plane’s Digital Flight Data Recorder (DFDR) and Cockpit Voice Recorder (CVR) were successfully retrieved from the seabed on 28 August 2007.

According to the final report by the Indonesia’s National Transportation Safety Committee, released on 25 March 2008, it was a pilot error. He became preoccupied with troubleshooting the aircraft’s navigation system, and did not detect and appropriately stop the descent soon enough to prevent loss of control.
